You understand you are providing your contact information to Hoag Urgent Care, and such information will be used for the purposes of recording participation in the Sweepstakes and winner notification.

ELIGIBILITY: OPEN ONLY TO LEGAL RESIDENTS OF THE STATE OF CALIFORNIA WHO ARE EIGHTEEN (18) YEARS OF AGE OR OLDER AS OF 10/31/2020. Employees, associates and contractors of Hoag Urgent Care, 2995 Red Hill Ave., Ste. 100, Costa Mesa 92626 USA (“Sponsor”), affiliate companies, and their immediate family (spouse, parents, siblings and children) or persons residing in the same household as they do are not eligible to participate or win. PROMOTIONAL PERIOD: Hoag Urgent Care Halloween $100 Gift Card Sweepstakes (“Sweepstakes”) begins at 9:00:00 a.m. Pacific Time (PT) on 10/31/2020 and ends at 4:30:00 p.m. PT 10/31/2020 (the “Promotional Period”).

BY COMPLETING ONLINE RESERVATION & ATTENDING DRIVE-UP TRICK-OR-TREATING: To enter complete the ONLINE RESERVATION form that is provided at HoagUrgentCare.com/Halloween/; and participate in the drive-up event. No other forms of entry or participation are valid. There is a limit of one (1) entry per person during the Promotional Period. Any attempt by any entrant to obtain more than one (1) entry by using multiple/different e-mail addresses, identities, registering for more than one event location or any other methods may void that entrant’s entries and that entrant will be disqualified. Entries must be received by 4:30p.m. on 10/31/2020 by Sponsor to be eligible for the random drawing. No automated entry devices and/or programs permitted. All entries become the sole and exclusive property of the Sponsor. Receipt of entries will not be acknowledged and entries will not be returned.

RANDOM DRAWING: Three (3) random drawings will take place on or about 5:00 p.m. on 10/31/2020 from all eligible entries received during the Promotional Period. One (1) drawing will occur for entries from each of the three locations. No substitution, cash redemption or transfer of prize, but Sponsor reserves right to substitute prize of equal or greater value if advertised prize becomes unavailable for any reason. By entering the Sweepstakes, entrants fully and unconditionally agree to be bound by these rules and the decisions of the judges.

PRIZES/ODDS: Three (3) Prize will be awarded. The Prize consists of three (3) $100 Gift Cards. Winner will receive gift card by email. Total Prize Approximate Retail Value (ARV): $100.00 per winner, for a total of $300. Winner is responsible for all taxes and fees associated with prize receipt and/or use. The odds of winning depend upon the number of eligible entries received at the end of the Promotional Period; i.e., if 270 entries are received at each location, the odds of winning are at each location is 1:270.

PRIZE AWARD/NOTIFICATION: Potential Prize Winner will be contacted on or about 11/2/2020 by e-mail and/or phone at Sponsor’s sole discretion, and will be required to complete and return an Affidavit of Eligibility and a Liability/Publicity Release (“Affidavit”), (unless prohibited by law). The Affidavit must be returned within five (5) days of receipt of the document. In completing the Affidavit the potential winner will be required to provide his/her social security or tax ID number, which will be used solely for the purposes of Sponsor’s legal compliance in filing US Tax Form 1099. Ineligibility or non-compliance with any of these requirements or return of prize, prize notification and/or Affidavit as undeliverable will result in disqualification, and forfeiture of the prize, and the selection of an alternate winner through random drawing in the manner described above.

CHOICE OF LAW: This Sweepstakes is governed by the laws of the United States and the State of California applicable to contracts made and performed in the State of California, without respect to conflict of law doctrines.
