Hoag Urgent Care is pleased to offer you immediate access to your test results and medical records through Hoag Connect MyChart Patient Portal.

The Hoag Urgent Care patient portal is easy to set up and provides you access to:

• View test results
• Review your urgent care visit summary and care instructions
• View your prescriptions
• Update your personal information
• Communicate with your provider
• Request prescription refills from your primary care provider
• Schedule appointments with your primary care provider or specialists

…all from the convenience of your laptop, computer or mobile device.

1. Create Your Hoag Account

• If you have a verification code, go to www.hoagconnect.org, and select “Sign Up Now”.
• If you do not have a verification code, go to www.hoagconnect.org/mychart/signup

2. Get The App:

You can access your Hoag Connect account, by downloading the Epic MyChart app and selecting Hoag Connect as your organization:

*Note: If you use Epic MyChart app for Hoag Hospital, this is a separate organization. You will still need to create your Hoag Connect account and link it within the Epic MyChart App.

Your health information and privacy are protected Only you, your provider and your health care team are permitted to view the information sent through MyChart. We use the latest security practices and safeguards to protect your health care information. MyChart is password-protected and delivered via an encrypted connection. PLEASE NOTE: Certain tests require a return visit to get results, including STD testing. If a return visit is required, your provider should have instructed you when to return.
