IMPORTANT INFORMATION

1. Many insurances do not charge co-pays for preventative services – please check with your insurance company if you have questions about your coverage.

2.  At this time we are unable to accommodate patients with a history of abnormal pap or mammogram results. Please visit your primary care provider, your ob/gyn, or contact us for a referral.

3. Orders are based on provider evaluation and will be customized based on individual medical need.

4. If you have had a previous mammogram at a facility other than Hoag, please bring a copy of your previous mammogram images on CD or a report.

5. The day of your visit, do not use deodorants, antiperspirants, powders, lotions, creams or perfumes under your arms or on your breasts. Metallic particles in powders and deodorants could be visible on your mammogram and cause confusion.
